Position: Licensed Practical Nurse (LPN)
Job Summary: This position provides high-quality client care to adults with identified mental health or substance use diagnosis. This position reports to the Director of Nursing.
Duties and Responsibilities:
Duties include, but are not limited to the following:
  • Monitor client health including vital signs, treatment plan progress, and activity participation
  • Collect objective and subjective client data, report symptoms, reactions, treatments, and changes in client's conditions to the RN or provider.
  • Prepare and give injections, collect samples for testing, and report adverse reactions to medication or treatment in accordance with policy
  • Utilize knowledge of medications (psychotropic and other) in the treatment process and educate clients and residential staff regarding medications and client health care needs.
  • Address signs and symptoms associated with substance use and behavioral health conditions
  • Screen clients for psychoactive substance toxicity and intoxication, danger to self or others, co-occurring mental health issues
  • Record all findings and consults with medical and clinical team when necessary
  • Maintain a comfortable, orderly, safe, and clean environment for clients
  • Record all care documentation concisely, accurately, and completely within the client electronic care record (ECR) and any other forms required by Lighthouse policy.
  • Participate collaboratively with the Lighthouse interdisciplinary treatment team and other agencies involved in the client's care as needed.
  • Coordinate client care activities, including scheduling appointments, arranging treatment, and ensuring follow-up.
  • Actively participate in required agency training, quality assurance initiatives, and continuing education required for state licensure
Required Experience/Certifications:
  • High School Diploma or equivalent.
  • Current and valid Ohio LPN licensure
  • Minimum of 2 years of experience as an LPN
  • Proficiency in using electronic care record (ECR) systems and other healthcare technology.
Desired Experience/Certifications:
  • Previous experience in behavioral health
  • Understanding of confidentiality obligations and nursing best practices
  • Excellent interpersonal skills with clients, colleagues, and community resources
  • Excellent time management skills; organized and able to prioritize

Monday - Friday 8:30am - 5p
