Job Description
Job Summary:
Administers care and education to designated patients per request of physicians or nurses. Provides resources, education and leadership to the nursing staff and communicates interdepartmentally.
Essential Job Duties:
  • Assess patient need for self care instructions in diabetes, wound care, ostomy care, bowel and bladder training.
  • Collaborates with nursing staff to develop the plan of care for patients requiring education and/or intervention in diabetes, wound, ostomy, bowel and bladder care.
  • Initiates instruction and promotes self care.
  • Implements strategies to provide patient, significant other, and staff participation in health promotion.
  • Utilizes age specific criteria in prescribing appropriate action to meet the needs of patients.
  • Completes documentation in electronic medical record, including target assessment, treatment plan, implementation and evaluation of status and outcomes.
  • Maintains communication both intra and interdepartmentally for continuity of care and patient education.
  • Coordinates and manages department Performance Improvement as well as educating all stakeholder on process and corrective actions.
  • Seeks educational resources and creates learning experiences to enhance and maintain current knowledge and skills for his/her competence in nursing practice and individual professional growth.

Job Specifications:
Education
  • Graduate of an accredited school of nursing
  • Current Registered Nurse licensure in the state of Indiana
  • Three years of nursing practice and teaching experience
  • Bachelor's degree preferred in Nursing.
  • Wound Care certification highly preferred.
  • Computer skills including (Microsoft, PowerPoint, Word, Excel).

Secondary Job Duties That May be Reassigned:
  • Completes other duties and special projects as assigned.
