Overview

Provide culturally competent, evidence-based nursing in community settings. The VNACJ Mobile Outreach Clinic Program (MOCP) aims to enhance the well-being of county residents who are homeless, have substance use disorders or mental illness, and are developmentally disabled, victims of domestic violence, or older adults. In collaboration with the Prevention Resource Network team, the nurse will focus on overdose prevention, naloxone education, and other harm reduction activities to mitigate drug-related harms and improve health outcomes while removing barriers to care.

Responsibilities
  • Foster the organization’s mission by excelling in all job aspects and cultivating positive interpersonal relationships with co-workers.
  • Monitor and ensure the quality and appropriateness of services in alignment with agency and contract guidelines.
  • Establishing trust, addressing and acting upon complex social and health care issues, advocating for people in high-risk situations, and intervening to prevent a problem from becoming a crisis are among the most significant services MOCP provides
  • Facilitate referrals and linkages to primary care and support services by building and sustaining community relationships
  • Assist with delivery safer injection education and overdose prevention training with PRN and Harm reduction team.
  • Complete documentation in accordance with agency guidelines.
  • Follow agency infection control and safety policies.
  • Comply with the organization’s policies on absenteeism and appearance
  • Qualifications
  • RN licensure for New Jersey
  • 2+ years of community health nursing experience; bilingual preferred
  • Strong Computer skills
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills with demonstrated ability to communicate with outside personnel
  • Ability to travel with a valid NJ driver’s license required
