PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :
- Responsible for collecting home care referral information and entering data into the EMR
- Assists with phone management and customer service
- Assists with scheduling on the weekend
- Assists with following/tracking active clients that have been transferred inpatient
- Works closely with field community liaisons to track/monitor referral activity
- Communicates daily with internal staff to obtain necessary data for dissemination to referral sources (branch availability by discipline, territory)
- This position is full time and the schedule is Monday through Friday with occasional weekend coverage as needed, as well as 2-3 major holidays per year.
- Maintains confidentiality of patient health information
- Adheres to OVNA and MHS policies and procedures
QUALIFICATIONS :
Must have excellent organizational, communication, and interpersonal skills
- Computer proficiency required
- Previous experience with HomeCare HomeBase preferred
- Knowledge and understanding of medical terminology strongly preferred
- Licensed LPN in the state of MA strongly preferred
- Experience in Customer Service or Hospitality preferred
- Must be able to work in a fast paced environment
- High School Diploma or equivalent
- Excellent verbal and written communication skills
- Ability to read, write and speak the English language
Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 35 pounds.
