We are seeking a compassionate and skilled Hospice Social Worker to join our team at Gentiva Hospice. In this role, you will provide essential psychosocial support to patients and their families, ensuring they receive the care and attention they need during a challenging time.
- Conduct psychosocial assessments and case management for patients and families
- Offer expertise in social work related to changes in the patient's level of care
- Provide casework and service management for patients and families, including long-term care planning and advanced care planning
- Monitor the safety of the patient environment
Qualifications:
- Master's Degree in Social Work from a CSWE accredited School
- Licensure as required by the state in which the hospice is located
- One year experience as a Social Worker in a healthcare and/or hospice setting is required
- Knowledge of terminally ill patients and their families
