JOB SUMMARY :
As a Hospice Aide you will perform personal care activities related to the activities of daily living, under direction of a Case Manager [RN].
QUALIFICATIONS:
- Must have reliable vehicle.
- 12 months of caregiving experience required (home care, assisted living/memory care, skilled nursing/nursing home, hospital or hospice),
- Hospice experience preferred
- Excellent customer service skills
- Current CPR Certified
- Valid driver's license and auto insurance
RESPONSIBILITIES (including but not limited to):
- Personal cares (bathing, dressing, grooming, toileting, skin care, etc)
- Review patient care plan
- Lifting and transferring
- Assistance with range of motion & positioning, exercise & ambulation
- Assistance with dining & nutrition
- Electronic charting
In addition:
- Needs strong understanding of professional boundaries
- Must be able to lift 50 lbs
- Good understanding of body mechanics
- Strong communication skills-written, verbal, listening with attention
- Good problem solving skills
- Good time management of self and others
- Able to function in an electronic medical record
