Job Description:
The Hospice Admission Nurse plans and delivers appropriate palliative & hospice care to patients through assessment, intervention, implementation, and evaluation while effectively interacting with patients, family members/significant others, and interdisciplinary team members to maintain standards of professional nursing and clinical competency.
Essential Duties & Responsibilities:
- Determine the appropriateness of referrals, coordinate and conduct
- Describe and explain hospice services to a potential patient/family and
hospice care, physical and psycho/social/emotional status in order to put
the patient and family at ease about choosing hospice and fully inform
them about the program;
- Gather pertinent information necessary for admission;
- Speak to patient/family about insurance coverage and financial obligations,
- Promote hospice services to ensure appropriate referrals to hospice
referral sources;
- Manage care of patients, coordinate all services and facilitate
- Provide skilled nursing care and prescribed treatments to patients and
- Maintain productivity expectations.
- Provide documentation of services utilizing problem-oriented record.
- Plan for continuity of care with nursing team members, patient, family, and
- Use opportunities for teaching nursing care, comfort measures and health
- Instruct and supervise care given by the Home Health Aides.
- Communicate and coordinate with other health and social agency
patient and family;
- Complete Hospice Item Set (HIS) time points. Accept accountability
results;
- Initiate the plan of care, as assigned, and make necessary revisions as
- Administer medications and treatments as prescribed by the physician;
- Counsel the patient and family in meeting patient needs related to physical,
- Provide information to the patient and family regarding the dying process
What You'll Need:
- BSN strongly preferred.
- Current valid license to practice as a registered nurse in Massachusetts.
- Current Basic Life Support Certification
- Valid Massachusetts driver’s license
- 1+ year of recent professional nursing experience
- Hospice or end of life care experience preferred.
