*Position Summary:* A *Health Care Coordinator (HCC) *is primarily responsible for assisting CHA Members and their families in achieving recovery by empowering members, assessing needs, identifying opportunities for service engagement, and encouraging members to establish and achieve recovery goals Duties include intensive case management, coordination of integrated care services, direct care activities in realms including employment support, health promotion, motivational interviewing, assessment & service planning, skills training, medication training, group facilitation, and other similar services to support recovery.

*Qualifications Required:*

· Bachelor ‘s degree in any field; or High School Diploma or GED with 2 years of behavioral health experience.

· Must be 21 years of age or older.

· Demonstrate the ability to provide safe, timely delivery of counseling support services, programs and activities as specified within Individual Service Plan as directed, supervised, or guided by the Clinical Liaison or Clinical Director.

· Demonstrate knowledge on direct management activities for behavioral health care of enrolled member/participants including service coordination for members in crisis, crisis intervention, behavioral health triage and other assessment, treatment, rehabilitation, and support services.

· Demonstrate the ability to review treatment plans and consults with Clinical Director and/or Clinical Liaison regarding medical necessity criteria; facilitates community placements, agency linkages and otherwise coordinates discharge planning, aftercare, referral, and follow-up.

· Demonstrate the ability to provide member care as directed by the Team Lead or Director in a manner that considers member rights (R9-20-203), cultural differences, safety, cleanliness, and comfort while promoting member dignity, independence, individuality, strengths, privacy, and choice

· Expand outreach activities and referral networks to ensure effective partnerships are formed which will facilitate the development of a comprehensive and geographically dispersed integrated behavioral health care services for members.

· Maintain a working knowledge of the mental health, developmentally disabled, social service agencies, and WRAP system in Yuma County to provide appropriate referral and linkage for members served; maintain and increase case management skills through completion of required training.

· Provide member care as directed by the Team Lead or Director in a manner that considers member rights (R9-20-203), cultural differences, safety, cleanliness, and comfort while promoting member dignity, independence, individuality, strengths, privacy, and choice.

· Ability to conduct out-of-office visits with members in homes or in the community, attend meetings, and work-related local travel using personal vehicle.

· Protect and maintain the confidentiality of member records and information.

· Certification in First Aid and CPR which Company will provide if candidate does not currently have in possession.

· Must be able to obtain and maintain a Level 1 Fingerprint Clearance Card and pass a background check.

· Possess valid driver’s license with acceptable driving record and reliable transportation.

· Working knowledge/familiarity with computers and electronic clinical record keeping.

· Able to effectively employ analytical and problem-solving skills.

*Qualifications Preferred:*

· Bachelor’s Degree in related field

· Bilingual – English/Spanish

· Experience working in the field or combined field specific education and experience totaling 4 years of experience.

· Advance knowledge of cultural diversity and sensitivity

· Independent License AZ BHBE (salary differential based on Licensure)

3. Demonstrate knowledge of the principles of instruction/training.

4. Demonstrate knowledge and/or ability to employ appropriate motivation techniques with SMI members.

5. Demonstrate knowledge of the community resources typically needed by the SMI population.

*Substance Abuse* – Staff conducting assessments, providing treatment, or developing programs for persons with substance abuse/addiction issues can:

1. Demonstrate understanding of the comprehensive psycho/social influences and effects of dependencies.

2. Demonstrate knowledge of the natural history and clinical basis of dependence.

3. Knowledgeable about available treatment resources and their appropriate uses.

*Child/Family & Peer Support* – Staff conducting assessments, providing treatment, or developing programs for children, youth and families with are able to:

1. Demonstrate knowledge of the behavioral health needs of children, youth, and families

2. Demonstrate knowledge of the safety, risk, and infection control practices pertinent the working with children.

3. Demonstrate knowledge and/or ability to employ behavioral management techniques with children and youth.

4. Demonstrate knowledge of the community resources to build assist with development of external support systems for families.

Everyone’s treatment process is guided and monitored by a multi-disciplinary staff. All aspects of service delivery are monitored to assure quality treatment and ethical business practices.

Pay: $18.00 - $23.00 per hour
