Full-Time Social Worker (MSW)

We are hiring a   Full-time Social Worker (MSW) to provide psychosocial support for seriously ill patients and their families.  

Responsibilities:

• Performing psychosocial assessments,
• Education and completion of advance care planning,
• Supportive counseling, and assistance with obtaining needed resources, including placement. 

You will work alongside your clinical and operational team members to ensure the provision of high-quality hospice services.

Qualifications:

• Applicants with master’s degree in social work preferred; will consider BSW. 
• one (1) + years’ experience as a Social Worker in a healthcare and/or hospice setting is required; three to five years preferred.
