Palliative Care Registered Nurse - Advanced Illness Management

Illumia Health is seeking a registered nurse to join our AIM (Advanced Illness Management) Program - Palliative Care team. You will work with patients, families, and healthcare professionals to provide high-quality end-of-life care in both in-person and telephonic settings. Responsibilities:

• Conduct in-person/telephonic RN assessments and provide patient education
• Complete risk stratification assessment and observe patient visit frequency
• Coordinate care with palliative team members and communicate findings to healthcare providers
• Document clinical records and support care plan development and implementation
• Facilitate in providing and obtaining care for patients as needed
• Participate in evening/weekend call as required and conduct on-call services

Requirements:

• Registered Nurse with 2 years of prior skilled nursing experience
• Experience in palliative care, oncology, hospice, or geriatric care preferred
• Current RN license and CPR certification
• Proficiency in computer programs and electronic medical record documentation
• Strong compassionate phone skills and good problem-solving abilities
