As a Clinical Registered Nurse at Mendocino Community Health Centers (MCHC), you'll be the heart of our patient care team, providing top-notch nursing care both in-person and over the phone. From administering treatments to lending a compassionate ear, your role will be pivotal in ensuring our patients receive the high-quality care they deserve.


Duties Include:

  • Actively contributes to the implementation of the "Health Care Home" model by engaging in improvement initiatives to prioritize the completion of clinical outcome measures.
  • Ensure follow-up for all patient ED visits, organize care for discharged hospital patients, collaborate with the care team, assess if additional follow-up is needed, and meticulously document all information in the patient's chart.
  • Collaborates closely with the Population Health team to bolster chronic care management efforts by identifying, contacting, and scheduling patients who are overdue for screening exams, diagnostic tests, immunizations, or other health maintenance tasks.
  • Manage tasks in personal and assigned providers' inboxes, ensuring timely completion based on team roles and responsibilities.
  • Provides vaccine and injection verification to the Care Team as needed for safe and accurate administration.
  • Attend and actively participate in meetings and quality improvement activities as necessary.
  • Acts as a specialist in nursing procedures, supporting the Nursing Leadership Team in the effective training and onboarding of staff. Additionally, contributes to educational efforts as required.
  • Participate in the daily narcotic count, ensuring accurate tracking and accountability of controlled substances.
  • Assists in the review, development, and implementation of clinical policies and procedures when needed.
  • Follows Universal Precautions, OSHA standards and is familiar with reporting adverse conditions including injuries and occupational exposure.
  • Travels to other sites as needed.
  • Other duties as assigned.

Requirements:

MINIMUM QUALIFICATIONS:

  • Valid State of California Certificate of Registration as a Registered Nurse
  • Satisfactory completion of a prescribed course of instruction in intravenous therapy, approved by California
  • Maintain current BLS certificate.

PREFERED QUALIFICATIONS:

  • Two or more years’ experience, as an RN, in an outpatient setting
  • Bilingual, English/Spanish, fluent

PHYSICAL REQUIREMENTS:

  • Must be able to move intermittently throughout the workday: 70% of time walking, 20% of time standing, 10% of time sitting.
  • Must be able to lift up to 40 pounds and push up to 100 pounds (on wheels).
  • Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
  • Must have vision that is adequate to read memos, registration forms and other clinic documents. Work may include extended periods of time viewing computer display monitor.
  • Must have high manual dexterity. Frequent repetitive hand and wrist motions.
  • Must be able to reach above the shoulder level to work, must be able to walk, bend, squat, sit for periods of up to 2 hours, stand, stoop, crouch, kneel, twist/turn, grasp and feel.
