Position Duties and Responsibilities:

  • Clinical Pharmacist shall provide services within the scope of the clinical privileges granted by MTF Leadership.
  • Provide Medical Therapy Management (MTM) services to eligible adult and pediatric patient populations for medical conditions that may include anticoagulation, diabetes, asthma, hypertension, hypercholesterolemia, and chronic pain.
  • Coordinate and collaborate with medical healthcare team members to assist with medication review to provide health maintenance and preventative services to individual patients as required.
Minimum Qualifications:
  • Doctorate degree in Pharmacy (PharmD) from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE).
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
  • Clinical Pharmacists shall possess one of the following:
  • Be a graduate certificate within the preceding year from an accredited pharmacy residency program.
  • Possess a minimum of 12 months of experience within the preceding 18 months as a Clinical Pharmacist performing direct patient care with clinical activities in an inpatient, outpatient, or ambulatory care pharmacy setting.
  • Have held clinical privileges as a Clinical Pharmacist within the preceding 36 months in a military treatment facility.
  • Must be a US Citizen.
