Required: MUST HAVE Epic and Careport experience MUST HAVE current acute care case management leadership experience (2 year minimum required) MUST HAVE inpatient hospital case management experience MUST HAVE discharge planning and utilization management experience MUST HAVE case management experience MUST HAVE Full License to practice as a Registered Nurse in the State of Michigan or Full License (LMSW) to practice as a Social Worker in the State of Michigan. MUST HAVE Certification in Case Management (CCM) by the Commission for Case Management. MUST HAVE Bachelor’s degree in nursing, healthcare management or related health care field required or a Masters of Social Work required.
