The IDDT Case Manager coordinates all services necessary to improve the quality of life for people with co-occurring, severe mental health and substance use disorders by combining substance abuse services with mental health services. This position does require working in the community and performing home visits.

How you will change lives…

Case Management

- Ensure consumer involvement in the Person Centered Planning Process (PCP).

- Facilitate the implementation of Individual Plan of Service.

- Perform comprehensive, ongoing psychosocial assessments.

- Participate in legal proceedings including petitioning and testifying in clinical emergencies.

- Provide community based services.

- Facilitate the coordination of consumer intakes, discharges, and transfers.

- Communicate with all service providers in Treatment Plan.

- Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.

- Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.

- Provide referral to appropriate community resources.

You have…

- Bachelor’s degree in a human services field from an accredited institution required.

- LLBSW, LBSW, LLMSW, LMSW preferred.

- Prior case management experience in a mental health or rehabilitation setting preferred.

- Valid Michigan Driver’s License.
