Key Responsibilities:

Develop and implement care management plans that integrate physical and behavioral health needs, ensuring continuity of care and client empowerment.
Assist clients in accessing health and disability benefits, securing housing, transportation, education, employment, and community resources.
Provide preventive interventions and education on topics such as smoking cessation, nutrition, exercise, and stress management.
Offer crisis intervention services, collaborating closely with management and therapists to ensure client safety and develop effective safety plans.

Qualifications:

Meet the ODMHSAS criteria for Certified Behavioral Health Case Management II (CM II) or obtain certification within 60 days of hire.
Minimum one year in a mental health-related field or healthcare setting preferred.
Proficiency in MS Word and general computer skills; excellent verbal and written communication skills; ability to work effectively in a multidisciplinary team.
Valid Oklahoma drivers license, current automobile liability insurance, and CPR/First Aid certifications required.
