NATURE OF DUTIES:
The nature of the duties is to serve as coordinator for all Cardio@Home initiatives, including CardioMEMS, Vivify, and other remote patient monitoring devices. This position is the center point for outpatient care of patients with chronic medical issues such as Congestive Heart Failure (CHF), Acute Myocardial Infarction (AMI), and others related to chronic cardiovascular conditions. The purpose of this position is to serve as the liaison between patients, physicians, and our hospital partners to assure that patients with chronic cardiovascular conditions are cared for in an optimal manner. This employee is also responsible for all remote patient monitoring, quality assessment of patients, care management, caller registration, physician referral, scheduling appointments, patient satisfaction, and clinical outcomes such as CHF and AMI readmission rates. This position will require virtual, telephonic and clinic patient visits in order to ensure compliance with treatment protocols and to determine when additional medical intervention may be required to assure optimal outcomes.
SPECIFIC DUTIES :
  • Works with MCI Cardiologists, Nurse Practitioners, clinical staff and hospital partners with Cardio@Home services in the treatment of patients with CHF, AMI, and related chronic conditions.
  • Facilitates clinical management and continuity of care for Cardio@Home patients, by monitoring web based dashboards for Cardio@Home service lines.
  • Responsible for the overall Cardio@Home program crossing many departments, and service lines to follow all CHF and AMI care delivered. Ensures that all protocols and quality of care meet evidenced based guidelines.
  • Reviews medical records of Cardio@Home patients and analyzes data regarding the provision of care to ensure proper compliance and timeliness of care. Populates databases in various systems to assure that data is retrievable and that analysis can be performed.
  • Completes concurrent medical record reviews timely to ensure patient care meets all criteria of the program. Works with clinical staff to ensure patient care delivered meets current standards of care. Advises immediate supervisor of any issues with physicians or staff who are not following established medical guidelines.
  • Provides education and guidance to all MCI staff on Cardio@Home initiatives.
  • Provides feedback to immediate MCI Physicians, MCI Clinical Staff and supervisor on methods to improve compliance with Cardio@Home program initiatives.
  • Participates in community projects for education and marketing of Cardio@Home Program.
  • Assists the Virtual Care Center Leadership Team in presenting quarterly data for Cardio@Home initiatives.
  • Responsible for reviewing Cardio@Home Program performance measures and makes recommendations for improvement.
  • Participates in business development initiatives to implement new Cardio@Home programs.
  • Other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Graduate of an accredited Nursing school.
  • Current licensure with the IL State Board of Registered Nurses
  • Basic Life Support certification (BLS) and Advanced Cardiovascular Life Support certification (ACLS) are both required.
  • Excellent clinical knowledge and analytical abilities; which will be necessary to plan and evaluate ongoing care of cardiovascular patients in Cardio@Home program.
  • Excellent verbal and written communication skills.
  • Very well organized and self-motivated, with excellent prioritization skills.
  • Good computer skills as they relate to compiling data on the care of chronic patients. Proficiency in the use of electronic medical records is required.

Exhibits good communication skills with physicians, patients, team leaders, and co-workers at all times.
  • Exhibits whenever possible a harmonious relationship with other CIS employees in order to accomplish the duties and responsibilities of the position. While perfect harmonious relationships with all employees is sometimes not achievable, not more than an occasional complaint should be received by the team leader about the incumbent of this position.
  • The employee shall work and relate cooperatively with all other employees (internal customers) of CIS to assure optimum care for the patients of CIS and to achieve standards of care set forth by the CIS medical staff.
  • Adheres to the CIS Compliance Plan as it pertains to the above specific job duties. Uses best efforts to maintain compliance by following the CIS Corporate Compliance Plan, attending CIS compliance education, following medical documentation guidelines, and communicating concerns regarding compliance issues.
  • Performs accurate and complete documentation in the patient's medical record, when applicable. This includes documentation in both the paper record and the electronic medical record (EMR).
  • Work with the electronic medical record (EMR) staff to maintain quality and work towards future EMR development.
  • Maintains patient confidentiality according to the HIPAA standards of privacy and security.
QUALIFICATIONS FOR THE POSITION:
  • Graduate of an accredited school of Nursing.
  • Current licensure the Louisiana State Board of Registered Nurses
  • Basic Life Support Certification and Advanced Cardiac Life Support Certification are both required.
  • Effective Communication Skills
  • Desired:

• Advanced certification in field of Cardiology/Cardiovascular nursing
• Two years critical care experience
