Kids in Crisis is Connecticut's ONLY free, round-the-clock agency providing emergency shelter, crisis counseling and community educational programs for children of all ages and families dealing with a wide range of crises, including domestic violence, mental health and family problems, substance abuse, economic difficulties and more.


RESPONSIBILITES

Case Management:

• Ensure all UC residents receive full range of assessments and program services

• Maintain case management load of 4-8

• Oversee safe and timely release services to ensure compliance with ORR policies and procedures

• Accompany and transport UC residents to court and other appointments

• Develop Individual Service Plans for each UC resident

• Maintain confidential UC resident records / files.

• Participate in weekly UC case review and Clinical meetings.

• Meet weekly with UC Lead Case Manager to ensure success in program design and implementation

• Transport/accompany clients to appointments and placements. Out of State travel via, agency vehicles, train or air.

• Completion of agency trainings including but not limited to TCI, Med Administration, Phone Training, Crisis Intervention, Sexual Assault Crisis Training, ORR P&Ps, KIC P&Ps, UC program P&Ps


ESSENTIAL FUNCTIONS

• Ability to lift a minimum of 20 lbs.; ability to Stand and sit for long periods

• Other duties as assigned


QUALIFICATIONS

• English / Spanish fluency required

• Bachelor's Degree in Counseling or the Human Services field required

• Minimum 1-2 years post-graduate experience with clinical assessments and crisis intervention

• Ability to work with multi-disciplinary team and knowledge of community resources a plus.
