Major Duties and Responsabilities

 

1. Participates in and receives nursing reports upon reporting duty and obtain CNA Assignment for each assigned resident.

 

2. Check Accountability Sheet and Care Cards.

 

3. For new admissions:

 

  •  

  • Ensure the Resident´s room is ready.

  •  

  • Greet and escort Resident to room.

  •  

  • Make Resident comfortable.

  •  

  • Introduce Resident to her/his room.

  •  

  • Call bell system.

  •  

  • Transport Residents as necessary.

  •  

 

4. Give or assist Residents with dental and mouth care daily.

 

5.  Give or assist Resident with bath functions(i.e, bed bath, tub, shower, etc) Ensure equipment is cleaned between Residents.

 

6.  Aids in meeting therapeutic activity needs of the Resident (i.e. bed bath, tub, shower, etc).

 

7. Assist Residents with grooming(i.e. hair care, nail care, Male residents to be shaved as needed).

 

8. Assist Residents in ambulating and transfer actovoties.

 

9. Keep Residents dry (i.e. hair care, nail care, etc).

 

10.  Position bedfast Residents in correct and comfortable position and turn every two (2) hours and prn. 

 

11.  Provides Skin Care as instructed.

 

12.  Routine colostomy care.

 

13.  Takes and records temperatures.

 

14.  Collects spicements (i.e . urine, stool)

 

15. Weighs Residents per P & P.

 

16.  Provides range of motion exercises as directed and ambulation as ordered.

 

17.  Assists with Residents on precautions as directly by Nurse.

 

18.  Administers Pos mortem care.

 

19. Asnwers Resident's call lights and requests for assistance. Takes care of Residence's needs and/or relays need to Nurse in charge.

 

20. Maintains Resident's environment by making beds,  occupied and unoccupied  and removing unecessary items from room, etc.

 

21. Distributes trays and feeds and/or assists Residents in eating.

 

22. Assists Residents with and adheres to policy and procedure for adaptive feeding devices.

 

23. Accompanies  Residents on trips and to medical appointments outside  facility  as directed, in uniform, wearing name badge.

 

24.  Applies and monitors use of corrective/positioning appliances.

 

25. Reports all changes in Resident's condition or problems in the delivery of care to the Charge Nurse immediately.

 

26. Used the ID band to identify patients before administering or serving meals, etc, as necessary, Make certain each Resident has an ID band on every shift.

 

27. Records all entries on flow sheets and accountability record accurately and at the time care is rendered, notify Nurse if Bowl movements are small, I & O's nourishment given and signed for.

 

28. Assists the Resident in packing personal possessions when being transferred or discharged.

 

29. Checks Residents routinely for whereabouts and assures that cares needs are being met. checks all alarm and safety measures at begining of shift, during shift, and at the end of shift.

 

30. Completes laundry duties as assigned, mark soiled bag of linen with date and shift.

 

31.  Follows assignments in completing and performing assigned tasks reporting any difference or changes in resident.

 

32. Performs all assigned tasks in accordance with facility policies and procedures and as instructed by superiors.

 

33.  Reports all compaints and grievances made by the Resident immediately to nurse in charge.

 

34. Cooperates with interdepartmental personnel, as well as other facilities personnel to ensure that Nursing Services meets the needs of the Residents.

 

35. Creates and Maintains  an atmosphere of warmth, personal interest, and a calm environment throughout  the unit and shift.

 

36. Meets with the shift's nursing personnel to assist in identifying and correcting problem areas and/or the improvement of services.

 

37.  Adheres to uniform code, including wearing  ID badge.

 

38. Attends inservice as scheduled.

 

39. Understands P&P on use of restraints

 

40. Release restraints every two (2) hours, during meals or as instructed on CNA assigment.

 

41. Immediately notifies Nurse Manager/Charge Nurse of any missing Resident.

 

42. Keeps floors dry. Picks up any soiled human wastes. Reports spills immediately 

 

43. Understands cell phones are forbidden in facility.

 

44.  Keeps excess supplies and equipments off the floor. Stores in designated areas. Keeps room clutter free.

 

45. Signs for all care given, no blanks on Accountability Sheets.

 

46. Accepts re-assignments for Residents who's care takers have gone off-duty, breaks, lunch, dinner, etc
