Job Summary

The Solid Organ Transplant Transition of Care (ToC) Clinical Staff Pharmacist plays an essential role in the transition of care for UTSW's patients. This position will be primarily responsible for the medication procurement process for the solid organ transplant patients at hospital discharge. Responsibilities include, but are not limited to, the following activities:

  • Medication procurement process (coordination with specialty pharmacies, social work, outpatient coordinators)
  • MedAction Plans and medication teachings (initial/updates/discharge) and notes
  • Pill box education at discharge
  • Discharge medication reconciliation
  • Attending multidisciplinary rounds

Experience and Education

  • One (1) year with experience in a hospital setting required for an inpatient position; 1 year retail experience for retail position preferred.
  • Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy.
  • Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required and Texas license with TSBP within 30 days of date of hire.
  • Sterile compounding (IV) certification accredited by ACPE.
  • Current Basic Life Support (BLS) certification with course accredited by the American Heart Association (AHA) or American Red Cross (ARC). Advanced Cardiac Life Support (ACLS) with course accredited by the American Heart Association (AHA) or American Red Cross (ARC) will be required for inpatient positions prior to or within 6 months of employment.

Job Duties

  • Performs job responsibilities per organizational and departmental policies, procedures, performance improvement objectives, environmental/infection control standards, and state and federal laws and regulations.
  • Performs all computer order entries accurately, thoroughly, and timely. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and documents recommendations appropriately.
  • Reviews medication orders, prescriptions, and home infusions for therapeutic appropriateness, duplications, interactions, overall effectiveness, and safety. Seeks clarification if necessary and documents interventions or recommendations (i.e., ADR).
  • Consistently works (multi-tasks) and solves problems independently and effectively with minimal supervision. Takes charge when necessary and is looked to for direction. Makes the right decisions and uses good judgment. Is cross-trained and multifunctional, maintaining competency in multiple areas.
  • Participates in cost-effective use and selection of pharmaceuticals.
  • Communicate drug information (verbally and in writing) to physicians, nurses, etc., courteously and professionally. Assists in patient/family education and counseling regarding drug therapy and side effects. Also, effectively documents clinical activities, demonstrating cost savings and concisely conveying critical information required for follow-up by fellow pharmacists. Strives to improve patient satisfaction, as measured by Press Ganey scores.
  • Assists in supporting pharmacy technicians (ie med history technicians) with any clinical questions and/or guidance that may be needed to complete their tasks.
  • Identifies opportunities for improvement in the patient transitions of care process.
  • Actively enhances personal growth and development by participating in educational programs, in-services, and meetings. Actively participates in new employee orientation and training. Willingly takes on additional projects as needs develop.
  • Appropriately demonstrates openness to diversity within the work environment and consistently respectfully deals with all customers. Supports teamwork and cooperation with co-workers in daily activities. Readily volunteers assistance to help co-workers complete tasks.
  • Actively participates in the clinical program. Supports P&T initiatives by reviewing patients on high-risk medications, Collects applicable data/measures and communicates opportunities for system or team improvement opportunities. Participate on committees or workgroups as assigned.
  • Communicates effectively with patients regarding high-risk medications and provides a comprehensive medication management review for patients at high risk for re-admission.
  • Provides educational materials and in-service for medical, nursing, and pharmacy team members.
  • Assists with Joint Commission standards and pharmacy safety initiatives.
