CareLink is a non-profit, mission-based 501c3 organization that was founded in 1997 by non-profit post-acute and community-based organizations specializing in the care of older adults and adults with disabilities. These organizations include long-term care, housing, hospice, home care, assisted living and adult-day health. In addition, CareLink is a direct provider of a range of health and wellness services including rehabilitation (PT/OT/SLP), mobile dental, audiology, podiatry, psychiatry, brain health and wellness services. We keep individuals doing the things that they want to do, independently, in the place they call home.

We are looking for a full-time Physical Therapist to join our team in the Greater Providence Region! We offer schedule flexibility, great benefits, an amazing team, rewarding work, and much more! Sign-on bonus of $1,000 with $500 paid upon hire and $500 paid upon completion of 6 months of employment.

Essential Functions :

  • Fulfill all job functions of a staff Physical Therapist within the clinical scope of practice.
  • Diagnosing patients by observing their movements and listening to their concerns
  • Reviewing patients' medical histories and develops an individualized treatment plan
  • Outlining clear goals for patients and the expected outcomes of the plan
  • Using exercises, stretching, equipment, and hands-on therapy to manage patients' pain, increase mobility, and prevent further pain and injury
  • Recording patients' progress and modifying the plan of care if needed
  • Educating patients and family members about the recovery process
  • Responsible for initiating prescriptive care as appropriate.
  • Communicating within a multi-disciplinary team who are committed to the patients' well-being
  • Responsible for documentation of these evaluations and treatments, recording patient status, treatment response, changes noted, patient/family education, recommendations and other patient-centered actions taken.
  • Assists in the implementation of patient treatment, including overseeing Physical Therapist Assistant's as necessary.
  • Complying with federal, state, and local certification requirements.

ADMINISTRATIVE AND ORGANIZATIONAL DUITES:

  • Has a working knowledge of job duties, departmental policies, and procedures; takes ownership to incorporate them in the daily routine.
  • Independently organizes a schedule that reflects patient care priorities and facility needs and responsibilities.
  • Carries a caseload that reflects the facilities productivity standards.
  • Ensures that equipment is in working condition and meets departmental standards prior to patient's treatment.
  • Apply indications and contraindications according to patient diagnosis and standard operating procedures.

Requirements:

Education/Experience Required:

  • Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited physical therapist program
  • Valid Physical Therapist license in the state(s) where services are rendered.
  • Interest in working with all abilities including the intellectual and/or developmental disabilities (IDD) population
  • Must possess a valid and current driver license and auto insurance.
  • Current (or willing to obtain) BLS and/or CPR certification from the American Heart Association or American Red Cross

Compensation details: 41-47 Hourly Wage
