A. Professional and clinical services:
1. Assist in maintaining a safe, healthy environment for patient and family.
2. Provide personal aspects of care for the patient to increase level of comfort and maintain personal hygiene.
3. Provide in-home/nursing home continuous care/respite for patient's family and provide relief from total personal care responsibility when a patient or family crisis exists.
4. Report significant change in patient's condition to RN for follow through and appropriate intervention.
5. Personal care, i.e., bathing, oral hygiene, skin care, hair care.
6. Transfer of patient i.e., bed to chair, bed to commode, chair/commode to bed.
7. Ambulation and exercise of patient as ordered and assigned.
8. Assist with bowel function including use of bedpans and bedside commode.
9. Maintain a clean, safe and comfortable environment for the patient.
10. Meal preparation and related tasks as assigned and within scope of practice.
11. Feeding of patient as assigned.
12. Have an awareness of the emotional status of patient/family and be able to understand the emotional problems associated with the illness and disease progression.
13. Coordination of care with nurse case managers and rest of hospice team, report of notable change in condition to nurses or Director of Nursing.
14. Accurate communication of information in writing and electronic format.
B. Education and outreach:
1. The Certified Nursing Assistant will assist in public education presentations when appropriate.
2. The Certified Nursing Assistant will assist in volunteer training presentations as needed.
3. The Certified Nursing Assistant will participate in new employee orientation and training
4. The Certified Nursing Assistant will, at times, be expected to participate in community educational training programs
OTHER DUTIES
1. The Certified Nursing Assistant will be responsible for ensuring completeness of their patient's medical records.
2. Ensure all documentation is maintained as per Medicare guidelines and HOM policy.
4. Other assignments as seen fit by Hospice Director of Nursing, Administrator, Director or Unit manager.
ESSENTIAL JOB QUALIFICATIONS
Physical Requirements
•Heavy Work: Sitting/standing, exerting up to 100 lbs. occasionally and/or 50 lbs. frequently, 20 lbs. constantly
The minimum requirements of this position require this individual to:
•Perform repetitive tasks and/or motions
•Distinguish colors
•Hear alarms/telephone/tape recorder/normal speaking voice
•Have good manual dexterity
•Have good eye-hand- foot coordination
•Have clarity of vision with or without corrective lenses at all times
•Interpersonal skills
•Competent home nursing assistant care skills
•Skills in conflict resolution
•Interagency and referral skills and experience.
•Organizational skills
Other nonphysical requirements:
•Must have valid Current Driver's License
•Furnish own transportation
•Must maintain current automobile insurance coverage
•Completion of approved Nursing Assistant Program with Certification
•Demonstrates knowledge of an ability to apply sound nursing assistant principals to Hospice care
•A minimum of one year's experience in hospice, or experiential equivalent preferred
•Maintains current Montana CNA Certification, in good standing
