Job Summary:
- Provide comprehensive care for patients with acute and chronic illnesses
- Take full accountability for patient care and outcomes
- Lead the care team and engage with hospitalists and transitional care teams
- Participate in clinical meetings and quality management programs
Key Responsibilities:
- Independently provide care for older adult patients
- Lead the care team and engage with hospitalists and transitional care teams
- Participate in Super Huddle and Transforming Care Meeting weekly
- Actively manage the center and support recruiting and interviewing of additional providers
Requirements:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
- Current, active MD licensure in State of employment
- Minimum of 1-year clinical experience in geriatric, adult, or family practice setting
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred
