JOB TITLE: Certified Nursing Assistant (CNA) OR Licensed Practical Nurse (LPN)

DEPARTMENT: Hospice House

JOB SUMMARY AS CNA

Under a professional care plan established by the agency, Hospice Aides provide valuable supportive services to patients whose health status is unstable and/or who require skilled professional services. Hospice Aides provide personal care and specifically defined non-skilled health care services and provide a clean orderly environment for the patient. On cases assigned, Hospice Aides work under the supervision of an RN or LPN. Their service is given in combination with the Hospice Interdisciplinary Team.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures. Must be available for rotating schedule.

JOB SUMMARY AS LPN:

The Hospice House LPN contributes to the skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Must be available for rotating work schedule.

JOB REQUIREMENTS

Required to possess a Current Wyoming CNA certification OR a Wyoming LPN license (IV-certified preferred).

EXPERIENCE

As a CNA - Two years of Hospital/Skilled Nursing Facility experience preferred.

As an LPN - Two years of nursing experience required. Hospice experience preferred.

SKILLS

AS CNA - Applicant must have the ability to follow oral and written instructions, establish and maintain effective working relationships, function effectively as a member of the health care team, make simple reports and accept and utilize supervision.

AS LPN - Ability to work autonomously. Excellent assessment and communication skills.

REQUIRED CERTIFICATIONS

BLS (AHA)

Clean & Valid Drivers' License
