What We Are Looking For
Job Title: Overnight On-Call Triage LPN - Community
Schedule: Sunday - Saturday 4:30PM - 8:30AM
Location: Multiple positions, Orange and Sullivan County NY

This position is an exciting opportunity to provide care to our patients located in their homes in the Orange and Sullivan County NY area. This position works as a member of the hospice interdisciplinary team to provide support and to implement the Plan of Care initiated by the RN.
Requirements
  • Candidates must have a current NYS LPN Licensure with 1-2 years of recent medical surgical experience employed as an LPN in an institutional setting, home care, or skilled nursing facility
  • Graduate of an Accredited School of Nursing
  • Applicant must demonstrate basic knowledge of nursing theory and practice, knowledge of appropriate technology, knowledge of practice standards for area of responsibility, sound clinical skills, sound standards of practice, and positive communication skills.
  • Previous hospice/home care experience preferred.
  • Must have a valid driver's license with a clean, acceptable driving record
  • Reliable transportation required
