This job was posted by : For more information, please see: Job Type Full-time Description The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive community health nursing services to women and their families eligible for the NFP program. Nurse Home Visitors (NHV) are pivotal in transforming the lives of vulnerable babies, mothers, and families by implementing and maintaining the fidelity of the NFP model. The NHV is responsible for maintaining the highest standards in clinical nursing practice, adherence to the polices, procedures, guidelines, as well as the standards of NFP and of the implementing agency. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. Essential Duties and Responsibilities: Provides home visits, in-home and/or virtual, to women and their families eligible for the NFP Program. Adheres to nursing process and the NFP model of home visitation. Develops and maintains community relationships to promote public awareness and to support client referrals. Carries a caseload of 25-30 clients, when working as a full-time NHV.
