Job Summary: The emphasis of the position is to handle day-to-day patient care within the setting of a primary care health center under the supervision of an RN/Clinical Manager and provider.
Job Duties:
  • Initial triage of patients regarding urgency of presenting problems.
  • Locate patient in examination room and make available to the provider all medical records of the patient and any supplies or instruments that may be required by the provider.
  • Collect and record complete first order data (history and basic vital signs) before a patient is presented to the provider.
  • Assist provider as required in treatment or examination of patient.
  • Perform authorized lab tests and record information in designated files.
  • Perform EKG tests as required.
  • Keep all rooms supplied to needs of provider and staff.
  • Check rooms for cleanliness after each patient visit.
  • Maintain check on health center supplies and list items to be purchased.
  • Schedule diagnostic test for patients as per provider's order as needed.
  • Provide instruction to patients in areas of medications, diet, health maintenance and diagnostic tests.
  • Record and observe provider orders.
  • Prepare specimens for outside lab test.
  • Make notations in EHR.
  • Ensure patient comfort and well-being while at health center.
  • Assist provider with procedures as needed.
  • Administer medications to patients according to provider orders.
  • Prepare sterile materials for use as needed.
  • Dispose of expendable material according to sanitation guidelines.
  • Performing nursing tasks such as dressing changes and ear irrigation as ordered by provider.
  • Assist in care coordination which may include obtaining test and referral results and communication with various entities including but not limited to community organizations, health plans, facilities and specialists.
  • Proactively address needs of the patients and families through the concept of population management.
  • Effectively communicates with all patients with specific attention to the vulnerable patient population.
  • Provide self management support in the form of patient coaching and motivational interviewing when appropriate.
  • Acknowledges the importance of a team approach when providing patient care.
  • Communicates and coordinates effectively when providing patient care.
  • Actively participates in the medical home care team which is comprised of clinical and non-clinical staff working together towards a common goal: delivering quality care to our patients.
  • Maintain compliance with the organization's confidentiality policy in accordance to the Health Insurance Portability and Accountability Act (HIPAA).
  • Perform other duties as assigned.
Working Conditions:
  • Position requires manual dexterity for operating standard office equipment.
  • Hearing must be within range of normal human conversation.
  • Position requires the ability to maintain concentration and attention to detail for long periods of time in order to maintain accurate records pertaining to patient and/or account information.
  • Position functions within a normal office environment.
Qualifications:
  • Graduate of LPN Accredited Program
  • Must maintain appropriate state licensure
  • Good organization and ability to multitask.
  • Friendly personality with the desire to work with the public.
