Clinical Practice Coordinator- Post-Acute

We are seeking a Post-Acute Clinical Practice Coordinator who is responsible general administrative support for the post-acute providers and facilities.

Schedule:
Monday- Friday 8 am – 4:30 pm
This is a hybrid position with the potential to be completely in-person/on-site.

RESPONSIBILITIES
Provides administrative support and resolves administrative issues for Physicians and Nurse Practitioners as requested (scheduling meetings, faxing, mailings, scanning, etc.)
Supports the Post-Acute team with administrative tasks which will include tracking of data, reporting, communication/dissemination of information.
Supports Operations Team with project work and other related tasks including, but not limited to:
Operate office equipment, such as voice mail messaging systems, and use word processing, spreadsheet, or other software applications to prepare reports, invoices, financial statements, letters, case histories, or medical records.
Serves as receptionist to greet guests coming into the office.
Receives all faxes and distribute to appropriate persons.
Receive patient orders for provider signature and send back to the appropriate destination.
Receiving voicemails and send to the appropriate destination as well as returning calls when necessary.
Sorts and distributes mail.
Initiates History & Physical and progress notes for new admissions to the MedOne Skilled Nursing Facilities, Assisted Livings, and Independent Livings. This role will be responsible for the completion of 5-10 medical summaries daily. Steps to complete the H&P and progress notes include:
Access records from the discharging facility (OhioHealth, OSU, IPR, CliniSync) and note chief complaint, diagnosis, treatment, labs, imaging results, family history, surgical history, medications, allergies etc. Also, copy hospital admission and discharge notes.
Maintain OneConnect queues.
Field questions and e-mails/texts from all PAC doctors and nurse practitioners
Update facility census from facility EMR into OneConnect
Enter new patients and assist with RXNT duties as assigned or needed.
Maintain contact lists for each facility; build relationships with facility contacts.
Obtain access to facility EMR systems to easily look up patient data (when available)
Upload new patient face sheet to billing company and compile accurate and verified insurance information.
Train new Nurse Practitioners and Doctors on One Connect Documentation Tool.
Organizing and managing requests from providers, facilities, patients, and home care companies for home care patients as needed
Assist with EMR issues and EMR tasks as needed.
Performs other duties as assigned, including but not limited to, assisting with Quality Initiatives, Facility Educations, entering patient labs, and training new team members.
QUALIFICATIONS

Education: High school diploma or equivalent

Certification/Licensure:
Licensed Practical Nurse (LPN) or Medical Assistance (MA)
CPR Certification
Experience: Prior administrative support and skilled nursing facility clinical experience preferred

Highly organized
Ability to multi-task and prioritize.
Detail oriented.
Clinical knowledge
Familiarity with medical terminology
Ability to work independently with little to no supervision.
Ability to establish and maintain effective working relationships with providers, staff, and contacts outside the organization.
Able to maintain strict patient confidentiality and follow HIPAA rules and regulations.
Proficient use of Microsoft Office applications (Word, Excel) and MedOne proprietary EMR
Efficient and accurate typing skills
