Hospice Physician
Trellis Supportive Care is seeking a Hospice Physician to join our compassionate and dedicated Medical Staff Services Team in Winston Salem, NC. As both the oldest and one of the largest not for profit hospice organizations in North Carolina, Trellis Supportive Care has been dedicated to providing exceptional end of life care since 1979. This opportunity allows the Hospice Physician to practice the full scope of hospice and palliative care such as home hospice care, inpatient hospice care at the Kate B. Reynolds Hospice Home, community outreach, and educational activities related to hospice and palliative medicine.

Responsibilities:
Provides direct medical care to patients assuring that patients receive appropriate measures to control uncomfortable symptoms.
Acts as an integral part of the interdisciplinary group and attends IDT meetings as scheduled, acting as a medical resource for the team, guiding the work of those who provide direct clinical services (RN, NP, LPN, CNA), and assuring compliance with medical standards of care for hospice.
Certifies that the patient meets the medical criteria for hospice admission based upon available diagnostic and prognostic indicators, related diagnoses if any, current subjective and objective medical findings, current medication and treatment orders, information about the medical management of any of the patient's medical conditions unrelated to the terminal illness.
Re-certifying patients, as appropriate, for continuation of the Medicare Hospice Benefit at the appropriate levels of care.
Assures physician representation and participation in the development of a patient's hospice plan of care.
In conjunction with the hospice team, the Hospice Physician participates in family meetings to educate patients and families on medical conditions and the pros and cons of treatments or medications.
Consults with the patient's PCP as needed regarding eligibility for hospice care, pain and symptom control. Complements attending physician care.
Actively participates in the organization-wide Quality Assessment and Performance Improvement (QAPI) Plan.
Maintains current knowledge of the latest research and trends in hospice care and pain/symptom management.
Qualifications:
Licensed to practice medicine in North Carolina (MD or DO).
Minimum of two (2) years' experience in medical practice or recent completion of a hospice and palliative medicine fellowship.
Hospice experience required.
Board certified in own specialty area.
Board certified in hospice and palliative medicine strongly preferred.
