Job Location: Bryant, AR Position Type: Full Time Education Level: High school diploma or general education degree (GED); or equivalent combination of education and experience. Must hold Arkansas Practical Nurses License. Must meet clearance specifications through OLTC Registry check system. Must have a valid driver's license. Must be computer literate and capable of operating basic office machines. Some clerical experience is desired.

SUMMARY: Provides quality services to the resident clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Prepares/dispenses medications consistent with physician orders.
  • Maintains client medication records as indicated by Office of Long-Term Care (OLTC) and Young Homes Administration.
  • Records monthly/periodic medications in Pharmacy log.
  • Maintains First Aid/CPR certification and administers if needed, while on duty.
  • Acquires TB card and updates as required. TB Administration Certification be completed
  • Maintains valid driver's license.
  • Assists clients in administering medications and provides training during medication passes according to Individual Program plans.
  • Assists with clients on hygiene-showers, nails clipped, ears/nose hair trimmed, weekly body audits, etc.
  • Documents daily results on each Individual Program plans' objectives.
  • Documents BIR's, Marks reports, Nurse notes, etc....when necessary
  • Responsible for reporting to facility Administrator (or designee), any information and/or incidents reportable under OLTC guidelines.
  • Ensures communication is being implemented to all staff and other LPNs
  • Uses cart/laptop to ensure EMARS are being used correctly and efficiently
  • Attends all training required by FCC and OLTC.
  • Attends mandatory once a month training at Young Homes.
  • Performs other assigned duties as necessary for ongoing facility operations.
  • Serves as Driver for participant day trips to various state locations. Driving may be required on as much as a daily basis. Required to undergo periodic driving test and demonstrate proficiency in the operation of a motor vehicle, including passenger vans. Employee may also be required to drive his/her own personal vehicle at times, and maintain adequate liability coverage on any personal vehicle used to transport participants.
  • This is a safety-sensitive position.

MATHEMATICAL SKILLS:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY:

Ability to apply commonsense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

QUALITY ASSURANCE & COMPLIANCE: To adhere to the following and comply with all Quality Assurance's & HIPAA procedures:

  • The Code of Conduct and Compliance Investigation Procedures as read and discussed in the new employee training must be followed at all times.
  • Consumers' progress notes be maintained and completed daily or as needed on the prescribed forms.
  • Consumers' records must be maintained weekly, monthly, quarterly, and yearly. All information must be filed in consumers' records as needed and updated as prescribed by funding sources.
