Avera is currently seeking a Registered Nurse (RN) – MDS Coordinator (LTC) looking to deliver the best possible nursing care in Avera’s patient-focused settings.

Job Summary

  • The RN MDS Coordinator is accountable for coordinating the Resident Assessment Instrument (RAI) process to ensure that comprehensive assessments are completed timely, and that each resident has an appropriate individualized plan of care. 
  • The MDS Coordinator will be the facility resource for continued staff education in the RAI process. 
  • Completes intake assessments and displays the ability to analyze care needs as well as financial impact of potential admissions. 
  • Participates in QA activities. 
  • Assists with continued development and utilization of the EMR.

Education and/or Experience

  • Graduate from a CCNE or NLN accredited nursing program is strongly preferred.
  • Graduate from an approved nursing program is acceptable.
  • Bachelor’s Degree in nursing is preferred.

Licensure/Certification/Registration

  • Licensed as a Registered Nurse in the State of SD and/or meeting the requirements of Multi-state licensure is required. 

Work Schedule

  • Monday - Friday; 80 hours every 2 weeks
    • 8:00 am - 4:30 pm
