Job Description & Requirements Specialty: Clinic Discipline: RN Duration: 26 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Salary: $3,430.00/week Standard Shift: Days Weekly Schedule: 5x8 Shift Timings: 08:00 AM - 16:30 PM Description: Must have adult Home Health experience, including OASIS documentation. Minimum 3 years' Home Health leadership experience Required - 5 preferred. Participates in the development of standards which ensure safe and therapeutically effective service to patient/family. Provides oversight of all patient care services including but not limited to making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring that patient needs are continually assessed, and ensuring the development, implementation, and updates of the individualized plan of care. Consults with the Administrator in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of health personnel and caseloads. Makes staff, patient, and on-call assignments. Ensures that assigned personnel have demonstrated competency in necessary tasks or services. Ability to supervise, guide and develop nursing skills and performances. Oversees recruitment, hiring, evaluation and termination of agency personnel. Oversees institution, interpretation, and documentation of disciplinary action. Responsible for the orientation of new agency personnel. Assists in the planning and consultative needs of staff. Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state, and local laws. Assists in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service. Participates in the coordination of Agency’s services with the services of other community agencies. Manages the Agency’s ongoing Quality Assessment and Performance Improvement (QAPI) program. Must be able to perform nursing functions in times of staff shortage. Available to share on-call with nursing and other administrative personnel as needed. Assumes the same responsibilities and obligations as Administrator in the Administrator’s absence. Promotes the use of best practices and evidence-based standards of care to achieve optimal outcomes within the most efficient use of resource. Communicates information appropriately in accordance with agency policies and procedures. Assists with the implementation of improved work methods and procedures that contribute to program effectiveness. Participates in the survey process according to the procedures established by the agency. Complies with all applicable statutes, rules, regulations, Conditions of Participation (CoP), policies and procedures, standards of conduct. Practices effective communication and interpersonal skills to enhance positive teamwork and relationships both internally and externally. Enhances own professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops. Attends all mandatory in-services. Maintains confidentiality of patient information and the business of OPH. Understands and models OPH mission, vision, and values. Willingly performs other duties as assigned or requested. Minimum Degree Required: Bachelor of Science (or higher) in Nursing Board Approved Program Preferred: Masters degree in Business or Health Administration. Required Certifications & Licensure: Current Hawaii nursing license.
