We seek a full-time Lead Case Manager at the Point in Salt Lake City, Utah!!

The Point case management lead provides direct supervision, training, and mentoring of the case management team at The Point supportive housing facility. This position develops, monitors, and implements case management services as outlined in our grant/contract with The Point, Friends of Switchpoint, and Salt Lake County Behavioral Health. This position provides reporting on the outcomes outlined to the development director and community funders. The Point case management lead attends meetings in the community and with partner agencies to assist in coordinating programs and case management services. The Point case management lead assists in other administrative tasks and works in concert with the priorities of the Executive Director, the Senior Management Team, and the Board of Directors. The overall goals of this position are to provide high-quality care for clients, coordinate services between the case management team and The Point facility staff, and provide supervision and support for the on-site case management team.

Pay

is $45,000 - $55.000 a year Our

Case management experience is required Experience working with the senior population is preferred. A minimum of a Bachelor's degree in social work or related field is preferred Experience in the substance-abuse field is preferred Experience in the mental health field is preferred Duties include but are not limited to:

Train, supervise and support the case management team at The Point Approve ADP time cards for all supervised reports Assign clients to case managers and provide oversite on services Assign case management staff to fit program needs Supervise case management of clients Complete quality checks on staff for case management needs assessments and case plans Mediate any issues between clients and case managers Monitor budgets and implementation for all case management grants and programs Complete quarterly and annual reports for The Point case management grants Inform case management team on resources to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community services Provide case management services to clients, as needed Collaborate with The Point staff and outside agencies to deliver case management services Use internal tracking system to ensure proper, accurate, and timely billing Use the Utah Homeless Management Information System (UHMIS) database to enter case-management notes, client information updates, and other relevant client information
