Position Highlights:

• Patient Care:
  
  • Rounding in Skilled Nursing Facilities (SNF)
  • 10-15 patients per day
  • Responsible for H&P, admission, progress, transfer, discharge, and acute/follow-up
• Supportive Environment:
  
  • Work with an interdisciplinary team
  • Coordinate care transitions to home, independent living centers, assisted living, and/or memory care
  • Collaborate with hospice, palliative care, and home care agencies

Requirements:

• Board Certification Requirements:
  
  • Certified Nurse Practitioner (CNP), Family Nurse Practitioner (FNP), Advanced Practice Registered Nurse (APRN), or Physician Assistant (PA)
• Experience:
  
  • New graduates accepted
  • Must be action-oriented and enjoy working with older adults in skilled nursing and rehab
• On-Call Duties:
  
  • Yes, but no primary night call
• Contract Type:
  
  • Employee / W2
