Description
Greater Nashua Mental Health is looking for a Nurse Practitioner to join our team! The Nurse Practitioner provides direct psychiatric evaluation, management, and psychiatric supervision of all clients assigned to the caseload. Provides psychiatric/medical input and guidance to the team's management of clients during team meetings. Reviews the treatment plan and ensures that it identifies appropriate problems, goals, and objectives. Provides psychoeducation to patient and family regarding diagnosis, medications, and treatment plans.
Duties & Responsibilities:

  • Provides direct patient care by performing psychiatric evaluations and medication management follow up visits, with a 70% productivity expectation. Reviews records and collateral information in preparation of the visits, and ensures that 95% of PHQ-9 responses are addressed. Addresses measures of metabolic syndrome at 90% of visits.
  • Collaborates with nursing staff, medical support staff, and other disciplines to ensure that timely and appropriate patient care is delivered.
  • Participates in weekly team meetings, relevant CAFS meetings, and Medical Staff meetings.
  • Completes all documentation within 3 business days (with first day being date of service/completion).
  • Promotes the quality of patient care through participation in weekly, and as needed supervision with the CMO, and maintenance of nursing/medical/psychiatric education.
  • Manages daily requests for medication refills through Essentia Rx.
  • Returns all phone calls to patients, family, and/or team members within 48 hours.
  • Performs other duties or special projects as required or as assigned.
Requirements
Qualifications:
  • Master's degree in Nursing
  • APRN Licensed in New Hampshire and DEA Certificate
  • 1 to 2 years or related work experience
  • Knowledge of psychiatric diagnoses, medications, and treatments prescribed to clients.
  • Knowledge of relevant nursing principles and procedures.
  • Experience using electronic medical records (EMR) systems.
  • Knowledge of Microsoft Office products (Email, Word, EXCEL).
  • Strong collaborating skills, and an ability to work effectively both independently and as part of a team.
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
  • Excellent communication and interpersonal skills.
  • Ability to multitask, and be flexible with regard to workload and assignments.
  • Ability to exercise good judgment and sound decision-making.
