Join our team as an Acute Care Nurse Practitioner or Physician Assistant and make a difference in the lives of patients in need of high-quality care. We are looking for individuals with acute care experience in ER, Urgent Care, or Internal Medicine to join our team. As part of a clinical care team, you will deliver care in the patient's home through our innovative in-home care service lines, using critical thinking skills and evidence-based standards of practice to diagnose and treat complex illnesses. Your responsibilities will include performing comprehensive health assessments, providing therapeutic interventions, analyzing test data, educating patients and families, and effectively communicating with all individuals involved in the patient's care. With a focus on delivering compassionate care, you will play a crucial role in our mission to provide trusted, compassionate care to all in the comfort of home.

Qualifications:

  • Minimum of two years of experience as an Advanced Practice Provider in relevant areas
  • Graduate with an advanced degree from an accredited institution
  • Current unrestricted state license as a Nurse Practitioner or Physician Assistant
  • Nationally board certified
  • Current BLS required, ACLS certification preferred
  • Prescriptive authority and DEA
  • Valid driver's license with clean driving record
  • Ability to lift and carry equipment up to 50 pounds
  • Flexible to work evenings, weekends, and holidays, as needed
