Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. Program Description: The Baxter Safe Haven is a transitional housing residence for unsheltered adults. Goddard Riverside Community Center is committed to building community and enhancing the lives of vulnerable New Yorkers by meeting essential human needs.
Purpose of Position: The RN will play a crucial role at our Safe Haven, which serves as a temporary home to 74 unsheltered individuals. The RN will be responsible for delivering direct and comprehensive nursing care, overseeing and monitoring medication management, and promoting physical and mental health integration. This role is vital in ensuring the overall health and well-being of our residents, providing both medical and psychosocial support.
Schedule: Monday through Friday 9:00 am to 5:00 pm
Salary Range: $105,000- $115,000
Roles, Responsibilities, and Essential Duties
Direct Service
  • Facilitate patient care within Safe Haven.
  • Review medical documents and shelter referral packets.
  • Manage inventory for medical supplies.
  • Coordinate and schedule patient appointments.
  • Provide triage, arrange lab work, administer medications, and educate patients.
  • Support the administration of medication and evaluate patient responses.
  • Maintain and track medication deliveries.
  • Plan and coordinate care and interventions.
  • Participate in clinical case reviews.
  • Provide crisis intervention and first aid as needed.
  • Run client groups on health and wellness 1-2x monthly.
Care Coordination
  • Manage Janian onsite psychiatric and primary care schedules:
  • Add clients to the schedule when referral documents are provided
  • Sign in and sign out with treatment team members on their scheduled Baxter days
  • Ensure clinical team submits referrals to primary care/psych and facilitate follow up on recommendations from treatment team
  • Develop linkages with other healthcare providers and resolve insurance issues:
  • Gather medical information received from other appointments or hospital visits
  • Assist clinical team with obtaining medical records from hospitals
  • Find providers (outpatient, specialty, additional health care services) for clients based on provider recommendations, geography, and insurance coverage.
  • Assist with insurance authorization process
  • Consult E-PACES to determine client medical coverage when necessary
  • Track clients' Medicaid/Medicare certification to ensure recertification
  • Other duties as required
Qualifications/Educational Requirements
  • Bachelor's degree in nursing from an accredited School of Nursing.
  • Current license to practice as a Registered Professional Nurse in NY.
  • BLS certification upon hire.
  • 2-3 years of experience, with a preference for experience in mental health and addiction settings.
  • Strong clinical and technical skills, including phlebotomy and medication administration.
  • Excellent communication, interpersonal, and organizational skills.
  • Compassionate and understanding towards individuals experiencing homelessness.
Skills, Knowledge and Abilities
  • Ability to multi-task/work in a fast-paced environment
  • Proficiency in Spanish and English preferred
  • Understanding of housing first and harm reduction models
  • Experience with homelessness preferred
  • Strong organizational and communication skills
  • Ability to work as part of a team
Computer Skills: To perform this job successfully, an individual should be:
  • Proficient in Microsoft Office: Word, Excel, Outlook
  • Able to use or learn computer databases such as CARES, E-PACES
Physical Requirements & Work Environment
  • High concentration and frequent interruptions.
  • Frequent standing, walking, bending, and lifting (up to 20 lbs.).
  • Frequent periods of sitting at a computer workstation.
  • Office is set within the Safe Haven Residence.
