Green Tree School & Services in Philadelphia, PA is seeking a qualified  Transition Resource Educator to design & implement a comprehensive transition training program for adult life after high school with our special education students at Green Tree School. The Transitions Resource Educator will coordinate educational & supportive services necessary to implement each student’s Individualized Education Plan (IEP) with integrity and provide an appropriate transition program.  $5,000 SIGN ON BONUS!!

RESPONSIBILITIES

  • Utilize research-based, best practice interventions, programs, and strategies to implement vocational curriculum for student’s progress with transition goals.
  • Deliver classroom instruction in a meaningful and appropriate way to enable students to progress toward goals outlined in the IEP.
  • Design a comprehensive vocational assessment plan for each student to appropriately determine transition plan outcomes & support student awareness of the skills needed for life after school.
  • Collaborate with IEP Case Manager to develop IEP transition goals and benchmarks based on vocational assessment results.
  • Develop a vocational profile showing current strengths and weaknesses as related to general job skills and work habits for all students 14 and older.
  • Participate in team meetings and IEP meetings to address student needs for the purpose of developing an appropriate Transition Plan for students 14 and older
  • Prepare specific work plans to assist the IEP team in the development of on campus activities to address areas of weakness for students, identified in their individualized vocational profiles.
  • Lead professional development trainings for the education department to support IEP teams with developing skills associated with differentiated instruction to support pre-vocational skill development and collecting pieces of evidence for career readiness portfolios.
