Description

This is a full-time non-exempt (hourly) position. The incumbent is responsible for providing positive support and supervision to persons with developmental disabilities with the goal of helping them achieve their highest level of independence and quality of life. Requirements

Maintain formal and informal communication with immediate supervisor keeping him/her aware of activities and concerns within the program, following the chain of command. Serve as the subject matter expert on everyone on your caseload; coordinate service for each person; ensure services are person centered and updated as needed. Ensure individual services are offered and provided in accordance with the Code of Maryland regulations (COMAR) for Developmental Disabilities and WCDC policies and procedures. Maintain a working knowledge of services offered under the Community Pathways and Community Supports waivers and their parameters. Demonstrate an ability to apply this knowledge when working with individuals on developing their Person Centered Plans (PCPs). Attend all Person-Centered Plan (PCP) meetings of assigned individuals and complete/gather the necessary documents to ensure the plan is finalized; distribute PCP's and ensure implementation and follow through with goal documentation. Develop strategies to help clients realize their potential and individualize programs to provide for the needs of each person served. Serve as liaison between WCDC and Coordinators of Community Service, individual's families/guardian, etc.; foster a positive and professional working relationship. Ensure individuals' information is up to date and provide training to all staff members; disseminate information as appropriate. Maintain formal and informal lines of communication with assigned individuals; check-in with individuals regularly to ensure services are delivered in accordance with his/her person-centered plan. Be an advocate for your clients, striving to ensure that they are healthy, safe, and receiving the services they want and need in the least restrictive environment. Gather and review monthly data for individuals on your case load, complete quarterly reviews, and hold interim team meetings as the data dictates. Meet all State of Maryland and WCDC deadlines when completing and submitting paperwork, reports, or documentation. Understand and follow all safety guidelines in accordance with each client person centered plan, including but not limited to care plans, behavior plans, choking protocols etc. Communicate regularly with staff members providing support to persons served to ensure individuals needs are being met and the PCPs are being followed. Work cooperatively and professionally with other staff members, take initiative, propose new ideas and different ways of doing things and be receptive to feedback. Perform basic functions of direct support staff and fill-in when and where needed. Serve on assigned committees. Attend required trainings and staff meetings; maintain certifications as required by DDA and WCDC. Perform other duties as assigned.

Qualifications:

Must be at least 21 years old, with a valid driver's license, and approved to transport clients. License may not be Provisionals and have a driving record that reflects responsibility and maturity. Complete all DDA Core Training successfully.

Education/Experience:

Bachelor's degree or equivalent experience in human services. Related experience may be considered in lieu of a degree.

Physical Demands/Work Experience:

Must be able to stand and walk 90% of work time, sit and/or drive 10%. Must be able to lift, carry, push, and/or pull 60 pounds, must be able to stoop, kneel, crouch and reach. All of the requirements are necessary to properly supervise and assist individuals with disabilities. Should be able to climb stairs/ladder and have good balance. Vision is required to drive.
