&lt;p&gt;The Wake Healthy at Home is a permanent supportive housing program which is federally funded and based in Wake County. The Tenancy Support Coordinator is responsible for case management and fostering an atmosphere of support and collaboration amongst landlords, developers, and providers to increase access to affordable housing and improve community tenure for the people we serve and are formerly homeless. Relationships with landlords are critical to the success of any housing program particularly working with individuals who may be chronically homeless, in need of permanent supportive housing, and access to support services, a housing voucher and/or subsidized housing option.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;*** This position is a provisional appointment meaning it is subjected to being annually renewed with grant funding.***&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;&lt;strong&gt;This is a full-time hybrid - remote opportunity. There is an expectation of being on-site in the corporate/home office occasionally, however, the selected candidate must be available to travel throughout the Wake County community as needed.  The selected candidate must reside in North Carolina. &lt;/strong&gt;&lt;/p&gt; &lt;p&gt;&lt;strong&gt;Responsibilities &amp;amp; Duties&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;Project Management of Supportive Housing Program&lt;/p&gt; &lt;ul&gt;&lt;li&gt;Track grant award expenditures to ensure compliance with HUD program guidelines and Alliance’s required match funds &lt;/li&gt; &lt;li&gt;Assist with the recordkeeping, documentation, and management of the Healthy at Home Program as it relates to meeting HUD compliance, performance metrics set by the Wake County Continuum of Care (CoC), and program guidelines &lt;/li&gt; &lt;li&gt;Connect the most vulnerable households in Wake County for permanent supportive housing according to the community established and HUD-approved standards for referral, administration, and service provision &lt;/li&gt; &lt;li&gt;Educate and provide technical support to community providers on referrals and adherence to program guidelines &lt;/li&gt; &lt;li&gt;Identify opportunities for housing advocacy and collaboration through community resources &lt;/li&gt; &lt;li&gt;Liaise with landlords and housing providers for rent and maintenance issues to ensure program participants live in safe, decent, and affordable housing &lt;/li&gt; &lt;li&gt;Provide case management to ensure all participants have resources available to meet basic needs for food, medication, access to services, and transportation &lt;/li&gt; &lt;li&gt;Assist and support participants in their housing readiness, sustainability, tenancy success, and graduating from the program to positive housing outcomes (when ready and available) &lt;/li&gt; &lt;li&gt;Enter data into the Homeless Management Information System (HMIS or equivalent) and provide accurate performance reports and analysis to Alliance staff, HUD, and the CoC  &lt;/li&gt; &lt;li&gt;Engage and advocate for community-wide policies and procedures that impact chronically homeless members served by Alliance and the local CoC&lt;/li&gt; &lt;/ul&gt; &lt;p&gt;Landlord Engagement and Tenancy Support&lt;/p&gt; &lt;ul&gt;&lt;li&gt;Assist in the coordination of annual landlord events for recruitment, appreciation, promotion of &quot;Housing First&quot; and reducing housing barriers for persons with mental and behavioral health concerns  &lt;/li&gt; &lt;li&gt;Coordinate and/or attend annual Fair Housing Trainings in partnership with NCHFA and NC Legal Aide &lt;/li&gt; &lt;li&gt;Identify and partner with local landlords and property management companies to ensure housing needs for people with behavioral health concerns are adequately addressed in federal funding and permanent supportive housing programs &lt;/li&gt; &lt;li&gt;Ensure landlord compliance with Fair Housing laws and promptly report violations and advocate for tenant’s fair housing rights for &lt;/li&gt; &lt;li&gt;persons with behavioral and mental health conditions&lt;/li&gt; &lt;/ul&gt; &lt;p&gt;Member Engagement and Community Collaboration&lt;/p&gt; &lt;ul&gt;&lt;li&gt;Collaborate with Healthy at Home Program Participants and applicable service providers to develop and implement plans for increased self-sufficiency, improved health, and increased income &lt;/li&gt; &lt;li&gt;Develop in-depth knowledge of various housing and financial systems and provide consultation to Alliance clinical departments regarding available resources and barriers for individuals who are involved with various community public systems to support resolution of housing crises &lt;/li&gt; &lt;li&gt;Partner with local Housing Authorities to increase access to public housing and voucher programs &lt;/li&gt; &lt;li&gt;Partner with local CoC and governments to ensure housing needs for people with mental and behavioral health concerns are adequately addressed in federal funding and consolidated affordable housing plans&lt;/li&gt; &lt;/ul&gt; &lt;p&gt;Customer Service&lt;/p&gt; &lt;p&gt; &lt;ul&gt;&lt;li&gt;Maintain quality relationships with housing providers to leverage and establish quality housing for Wake Healthy at Home Participants &lt;/li&gt; &lt;li&gt;Respond to all voicemail messages, emails, and correspondence within 24 hours &lt;/li&gt; &lt;li&gt;Prompt follow-up with all referrals, inquiries, and emails from internal staff &lt;/li&gt; &lt;li&gt;Respond to all emergency situations with high level of urgency as needed&lt;/li&gt; &lt;/ul&gt; &lt;p&gt;&lt;strong&gt;Minimum Requirements&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;Education &amp;amp; Experience&lt;/p&gt; &lt;p&gt;Bachelor&#39;s degree from an accredited institution in a human services field such as social work, psychology, sociology, community development, political science, or public administration and three (3) years of experience in social work, human services, and case management preferably with the client population and at least one (1) year of experience in housing case management. &lt;/p&gt; &lt;p&gt;Or &lt;/p&gt; &lt;p&gt;Master’s degree from an accredited institution in a human services field such as social work, psychology, sociology, community development, political science, or public administration and one (1) year of experience in social work, counseling, preferably with the client population working in housing case management.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;Knowledge, Skills, &amp;amp; Abilities&lt;/strong&gt;&lt;/p&gt; &lt;ul&gt;&lt;li&gt;Thorough knowledge of effective homeless service interventions and HUD program guidelines, as it relates to homeless individuals with disabilities, as well as local community resources and service connections to ensure success in housing &lt;/li&gt; &lt;li&gt;Exceptional interpersonal skills, clear and highly effective verbal and written communication skills, active listening skills, and commitment to person-centered goals   &lt;/li&gt; &lt;li&gt;Adaptable to and accepting of people of diverse backgrounds and beliefs and have the patience to build rapport and relationships through ongoing engagement &lt;/li&gt; &lt;li&gt;Ability to maintain a non-reactionary and non-defensive communication style during challenging discussions &lt;/li&gt; &lt;li&gt;Skilled in critical thinking and solution orientation &lt;/li&gt; &lt;li&gt;Ability to work independently and triage competing complex needs of participants and the propensity to make prompt  decisions based upon relevant facts and ethical standards &lt;/li&gt; &lt;li&gt;Problem solving and conflict resolution skills are essential &lt;/li&gt; &lt;li&gt;Knowledge of behavioral health and intellectual/developmental disabilities and Social Determinants of Health &lt;/li&gt; &lt;/ul&gt; &lt;p&gt;&lt;strong&gt;Salary Range &lt;/strong&gt;&lt;/p&gt; &lt;p&gt;&lt;strong&gt;$22.23- $24.45/Hourly &lt;/strong&gt;&lt;/p&gt; &lt;p&gt;Exact compensation will be determined based on the candidate&#39;s education, experience, external market data and consideration of internal equity.  The selected candidate will be eligible to participate in Alliance&#39;s PPO Health Benefit Plan.
