The LPN is responsible for identifying patient/family needs, and for providing supportive care, in accordance with the attending physician's orders and the interdisciplinary group plan of care. The LPN reports to the Clinical Coordinator and Program Director for the provision of evaluation and care in order to maximize the comfort and health of patients and families consistent with Dependable Hospice Care policies and procedures.

We are currently seeking a self-motivated, dynamic, and caring individual to join us as a Hospice Licensed Practical Nurse (LPN) in the Nogales, Arizona area.

Qualifications:

Current licensed as an LPN/LVN in the State of Arizona

Some after hours and weekend coverage is a must

Demonstrates good verbal and written communication, and organized skills

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order

Possesses and maintains current BLS certification

Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles

Currently possess or able to obtain an Arizona class 1 fingerprint clearance card

Home Health experience a plus

Bilingual (English/Spanish) Strongly Preferred

Responsibilities:

Identifying patient/family needs, and for providing supportive care, in accordance with the attending physician's orders and the interdisciplinary group plan of care

With the assigned Nurse Case Manager, assume responsibility that includes the assessing, phases of the nursing process

Initiate communication with attending physicians, other staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family

Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director

Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and report to the assigned Registered Nurse Case Manager
