As a caregiver in the Caregiver Management Resource Center, you will absorb the non-clinical functions of the Care Management Team. You will initiate the post-acute perfect process with facilities and insurance companies, making referrals to post-acute agencies, and documenting the details of the discharge transition plan. Your primary duties will include: Completing data tracking and collection to assist with identification of routine statistics and problems. Supporting the Transitional Care Coordinator and Inpatient Social Worker with non-clinical work. Handling calls from commercial payor case managers with questions regarding patient discharge plans. Initiating contact with post-acute agencies and providing updates as requested.The ideal future caregiver is someone who: Has strong computer literacy skills. Can effectively multitask. Is self-motivated and efficient. Shows strong verbal and written communication skills and fluency in medical terminology. When you join Cleveland Clinic, you'll be part of a supportive caregiver family that will be united in shared values and purpose to fulfill our promise of being the best place to receive care and the best place to work in healthcare.Responsibilities:Support the Transitional Care Coordinator and Inpatient Social Worker with specified non-clinical itiate contact with payers to facilitate precertification for transfer to post-acute facilities, provide clinical documentation, and ensure timely follow up itiate contact with post-acute agencies, provide updates as requested, facilitate placement.Facilitate 7000 forms by Healthcare Electronic Notification System (HENS).Place and Close Referrals in AllScripts Care Management.Handle calls from commercial payor case managers with questions regarding admitted patient's discharge plans.Manage post-acute pre-cert process for commercial insurance patients needing pre-cert to post-acute facilities.Provide necessary verbal and electronic communication to and from care management staff, serving as a liaison between hospital personnel and the various agencies.Serve as a central hub communicator for discharge plan pletes data tracking and collection to assist with the identification of routine statistics and problems or trends,Other duties as assigned.Education: High School graduate or equivalent.Fluent in medical terminology.Demonstrated excellent verbal and written communication skills.Demonstrated ability to set priorities and good organizational skills.Must be able to work in a stressful puter literacy.Certifications:None plexity of Work: None required.Work Experience: Three to five years of current administrative/clerical experience working in Healthcare/and or related field.Physical Requirements:Ability to perform work in a stationary position for extended periodsAbility to operate a computer and other office equipmentAbility to communicate and exchange accurate informationPersonal Protective Equipment:Follows Standard Precautions using personal protective equipment as required.Pay RangeMinimum hourly: $16.47Maximum hourly: $23.61The pay range displayed on this job posting reflects the anticipated range for new hires.
