Job Summary:
We are seeking a skilled Social Worker to join our team at LifeBridge Health. As a Social Worker, you will play a critical role in providing direct services to patients and families to identify and resolve problems that impede discharge planning or the patient's use of healthcare resources.
Key Responsibilities:
- Assess and develop discharge plans for adult and geriatric patients.
- Collaborate with physicians, nursing, and other members of the multidisciplinary team to promote care coordination.
- Provide emotional, social, and financial support to patients and families.
- Identify and mobilize community resources to meet patient needs.
- Maintain timely and consistent documentation of patient interventions.
Requirements:
- Master's Degree in Social Work/Counseling from an accredited institution.
- Current licensure in the State of Maryland – LMSW/LGPC required; LCSW-C/LCPC preferred.
- Minimum of 1 year of experience in a healthcare setting; hospital preferred.
