Job Summary
We are seeking a skilled Social Services Coordinator to join our team at LifeBridge Health. As a key member of our healthcare team, you will be responsible for providing direct services to patients and families to identify and resolve problems that impede discharge planning or the patient's use of healthcare resources.
Key Responsibilities- Assess and develop discharge plans for adult and geriatric patients, taking into account their unique needs and circumstances.
- Collaborate with patients, families, physicians, and other healthcare professionals to ensure comprehensive care and support.
- Identify and mobilize community resources to meet the needs of patients and families, promoting a smooth transition from hospital to home.
- Document all interventions and services provided, maintaining accurate and timely records.
- Participate in multidisciplinary rounds and contribute to the development of care plans that address the social and emotional needs of patients.
- Master's degree in Social Work or a related field from an accredited institution.
- Current licensure in the State of Maryland - LMSW/LGPC required; LCSW-C/LCPC preferred.
- Minimum of 1 year of experience in a healthcare setting, with a focus on social services or discharge planning.
