We are seeking an innovative and entrepreneurial Senior Healthcare Provider to join our team at ChenMed, a leading primary care organization. The ideal candidate will be accountable for patient outcomes, act as a health coach, and simplify actions for improved patient health. The Senior Healthcare Provider will work closely with care teams, lead care coordination for patients, and engage in center management. Responsibilities:
- Provide care for patients with acute and chronic illnesses
- Assess, diagnose, and treat patients, and manage care coordination and documentation
- Lead care team and engage in center management
- Stay updated on evidence-based medicine and lifestyle-based interventions
- Engage with hospitalist when patients are in the hospital
- Work with care teams to achieve high quality outcomes
- Build trust with patients and provide accessible care
- Continuously work to develop and improve leadership skills
Requirements:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
- State Medical License for the state of practice
- Board certification in relevant specialty (preferred)
- Basic Life Support (BLS) certification
- Current DEA number for schedule II-V controlled substances
