Fort HealthCare is a patient-centered hospital and healthcare system serving individuals, businesses and families in Jefferson, Dane, and Walworth Counties, and we are seeking a Registered Nurse to join our Wound & Edema Center team.

The Registered Nurse is Responsible for the efficient flow and compassionate care of patients at the Wound Care Center. This includes, but is not limited to, utilization of skills in organization, planning, implementing, evaluating the provision of care through the use of recognized and approved hospital and nursing standards. Utilizes the nursing process (assessment, planning, intervention, and evaluation) in all encounters.
Responsibilities of this position include:

  • Performs all professional skills and duties with goal of meeting Meaningful Use, Universal Protocols, and Joint Commission guidelines.
  • Serves as case manager for patients provides assessments according to protocols and procedures; demonstrates clinical competence in procedural skills and treatments. Establishes teaching protocols for patient/caregiver education specific to wound care techniques; effectively communicates instructions at the individual's level of understanding. Acts as patient advocate in delivery and coordination of care.
  • Provides efficient and effective care with the guidance of the Physician and Clinical Coordinator during the clinic sessions. Able to manage care and treatment of numerous patients simultaneously with high energy, compassion, and organization.
  • Possesses knowledge of tests and procedures as they relate to wound & edema care. Recognizes significant values and works with the Center Panel Physician to notify the patient of results and arrange for follow up activities.
  • Communicates and/or completes provider orders to outside sources (i.e. pharmacy, other clinics, hospital, nursing homes, etc). Also assists patient in the completion of various medical forms (i.e. RTW, worker's compensation, surgical consent, etc.). Records complete and accurate patient medical data as appropriate in two systems. Completes charge ticket with all nurse rendered services; generates and completes ABI and related patient forms as appropriate.
  • Performs laboratory testing covered by CLIA such as venipuncture; prepares specimens for transport; complies with quality control measures. Assists medical provider with patient injections (draws medication, prepares injection site) processing test results; contacts patients with test results; conveys additional information per Center Panel Physician. As staffing needs dictate, may room patients, gather and review health history, address preventative maintenance issues specific to wound and edema care.
  • Collaborates with other health care providers, Center Panel Physicians, Program Director, and Medical Director, regarding clinic and patient needs. Effectively counsels and educates nursing professionals throughout system on advanced wound care techniques. Exp: Home Health, Inpatient Wound Consult Program.
  • Participates in development and implementation of new service-lines as requested. Analyzes progress and results; proposes necessary changes to Center Management. With Clinical Coordinator, researches products and supplies; makes recommendations for utilization; participates in training and implementation of new products. Works w/ vendors as directed and appropriate.
  • Assists in evaluating current Center processes and procedures, identifying and reporting areas for improvement. Adheres to HIPAA, TJC, DOH and hospital requirements including utilization of Clinical Practice Guidelines.
  • As trained, serves as a back-up with hyperbaric treatments when scheduled or when Hyperbaric Tech is unavailable; maintains hours to meet requirements and HBO chamber operator competencies.
  • Follows the FHC clinic decontamination and sterilization policy as appropriate to the specific clinic and equipment needs. Executes scheduled cleaning maintenance of Center equipment; initiates requests for repair (or replacement) of medical equipment to Center Management.
  • Performs other duties as assigned.
  • Completes department-specific orientation and annual competencies for this position.
Requirements for this position include:
  • Associates Degree in Nursing required
  • Bachelors Degree in Nursing preferred
  • One to two years related experience and/or training; or equivalent combination of education and experience required
  • Ability to effectively communicate, verbally, orally and in writing, in person and telephonically, with co-workers, physicians/Nurse Practitioners, patients, and patients' families; must interact well with co-workers required
  • Current Registered Nurse in the State of Wisconsin required
  • BLS certification required
  • Wound and Ostomy Certification preferred
