RN / Swing Bed Liaison

Deaconess Health System is looking for a compassionate, caring, and dedicated nurse to help us continue our tradition of excellence!
This position will play a key role in the growth of our swing bed program. The RN- Swing Bed Liaison will help strengthen and expand our referral patterns with outside facilities/ providers, will be responsible for welcoming and completing the readmission education packet, rounding on inpatient and outside referrals, and meeting with ED patients and families prior to transfer to educate on our swing bed program.
This person would also be responsible for the Swing Bed QAPI program, scheduling care plan meetings, updating the patient charts, attending weekly interdisciplinary team rounds, discharge planning, community education, and updating family and patients on their plan of care. This posltion will be working very closely with the case management team and filling in for case management in the event of an absence.
Required: Certifications /Licenses in IL or compact licensure states:
Active RN
BLS
