Physical Therapist (PT)- Home Health

Pioneer Healthcare Services is seeking a committed Home Health Physical Therapist to join our team in Oxnard, CA. The qualified HH PT will act as the students' partners throughout their daily tasks and assist students to function at their personal best. You will maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and of a Home Health Physical Therapist:• One year of home health experience preferred• Developing and revising the plan of care in consultation with the physician and other health care professions• Evaluate and treat patients recovering from injury, disease, or surgery in a home care environment• Preparing clinical progress notes, evaluating outcomes• Establishes treatment plans using a variety of methods• Able to respond to inquiries and complaints from employees and regulatory agencies• Meet all productivity standards and requirements daily• Must have a clean driving record to apply• 40 hours guaranteed• Submit all billing logs and necessary information on a timely basis• New grads and experienced therapists are encouraged to apply - We will support you through this transitionPioneer Healthcare Services is a San Diego-based staffing and travel recruitment firm that specializes in finding exceptional nursing and therapy positions for its clients across the US.
