Summary:
The Case Manager coordinates the care and service of the rehab patient population, promotes effective utilization and monitoring of health care resources and assumes a leadership role with the interdisciplinary team to achieve optimal quality, clinical, financial and resource outcomes.
Requirements:
Minimum 1-2 years experience in the acute care or rehabilitation setting, recent experience in the supervision and treatment of patients, prior experience in utilization review, discharge planning, PPS, quality management or case management preferred.
Knowledge of computer software and hardware application.
Skill and proficiency in applying highly technical principles, concepts and techniques that are central to the nursing profession, ability to be objective in stressful situations, advanced communication and interpersonal skills with all levels of internal and external customers.
This includes, but is not limited to, medical staff, patients and families, clinical personnel, support and technical staff, outside agencies, and members of the community.
Maintain a high degree of confidentiality and adapts easily to ever changing environment
RN OR BSNCPR Certification.
CPI Certification within one year of hire.
