Home Care Physical Therapist

The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages, modifying the plan of care as appropriate. This role also involves serving as a clinical resource for staff, students and other healthcare professionals, and serving as a role model in the delivery of professional services. The schedule for this position may vary depending upon census and program need.

  • Document patient visits with comprehensive detail, ensuring that following staff can determine precisely how care was provided and have a clear plan for the next contact.
  • Educate patients and their caregivers on home exercise programs and provide recommendations for improving home safety.
  • Perform patient assessment tests, measurements and evaluations, such as range of motion tests, gait analysis, and body part measurements, and record and evaluate findings to aid in the establishment or revision of treatment programs.
  • Communicate with patients to schedule appointments and manage daily schedule effectively, and coordinate and communicate with other healthcare professionals and caregivers as necessary.
  • Administer treatment involving application of physical agents and other modalities, as well as evaluate and adjust prosthetic and orthotic devices and recommend modifications as needed.
  • Complete comprehensive OASIS assessments and other required documentation in a timely manner, ensuring adherence to regulatory requirements and home care policies.
