Description
Job Duties:

1. Provide transition services for individuals with mental illness back into their communities with the goal of averting future mental health crises by utilizing Personal Outcome Measures (POM), Wellness Recovery Action Plans (WRAP) and community resources.
2. Utilize evidence-based practices where applicable and monitor Performance Outcome Measures to see if goals are being met. Provide services based on the Person-Centered Recovery model.
3. Decrease need for readmission into inpatient psychiatric care by providing support and working with the ICSS to coordinate needed community based services upon discharge from inpatient care.
4. With assistance from the Intensive CSS, link the person to a broad range of community-based services and natural supports as identified on the ISP and WRAP plan.
5. Increase the number of individuals who attend follow-up appointments by offering intensive peer support services including linkage between behavioral health programs and community mental health programs.
6. Meet with persons virtually or in person prior to hospital discharge to begin working on goals for transition to the home/community.
7. Meet with the person and their family members (with appropriate consents) to develop an individualized Wellness Recovery Action Plan (WRAP) plan. Solicit input from the person and family (when authorized) to develop the plan.
8. Share personal lived experience to establish rapport, build trust, and give hope that recovery is possible.
9. Assist individuals in articulating and determining objectives to reach personal goals for recovery, as well as, developing empowerment skills through self-advocacy and support.
10. Maintains client confidentiality at all times.

Documentation and Quality Improvement:

11. Document relevant events within specified timelines.
12. Document progress individuals make toward meeting objectives and personal recovery goals.
13. Complete and submit documentation in compliance with agency policy and DMH standards.
14. Submit monthly report on performance measures by the 15th of the following month.
15. Review data on performance measures monthly to determine if goals are being met.

Interpersonal and Leadership Skills:

16. Attend and participate in treatment team meetings as needed.
17. Assist non-consumer staff in identifying program environments that are conducive to recovery.
18. Share unique insight into mental illness with non-consumer staff and what makes recovery possible.
19. Serve as a role model and emphasize the value of each individual's recovery experience.
20. Respond to individual needs in an appropriate and timely manner.
21. Demonstrate appropriate interactions with coworkers that promote a pleasant work environment.
22. Participate in meetings/trainings with other Peer Support Specialists. Develop and share recovery-orientated material with other Peer Support Specialists.

Support Duties:

23. Complete additional job duties as assigned by supervisor.
24. Participate in interagency collaboration for the provision of services.
25. Maintain a working knowledge of current trends in the mental health field and effective peer support services.

Key Responsibilities
ESSENTIAL QUALIFICATIONS/SKILLS/EXPERIENCE:
Ability to be certified and complete DMH Certified Peer Support Specialist program
Demonstrate a minimum of six (6) months in self-directed recovery.
An individual who self-identifies as a consumer or former mental health consumer.
Completion of required training in compliance with DMH standards.
Requires satisfactory completion of CPR prior to reporting to the work site.
Requires satisfactory completion of CPI within 30 days of employment.
Requires valid driver's license and auto liability insurance.

PREFERRED QUALIFICATIONS/SKILLS/EXPERIENCE:
Previous college educational experience.
Previous paid and/or volunteer experience.
